INTRODUCTION:

This information has been prepared in accordance with Article 10 of the Law on the Protection of Personal Data No. 6698 (“Law”) titled “Data Controller’s Obligation to Disclose” regarding your personal data collected, stored and processed by A LIFE HOSPİTAL (“Our Hospital”) as the “Data Controller”.
Within the scope of our hospital’s understanding of carrying out all its activities in accordance with the principles of lawfulness, honesty and transparency, your personal data will be processed within the framework set forth by the Law and except for the cases where your personal data is processed and transferred to the extent necessary for the execution of the contract, if it is clearly foreseen in the law, if it is mandatory for us to fulfill our legal obligations and for the protection of public health, preventive medicine, medical diagnosis, treatment and care services, planning and management of health services and financing; we request your explicit consent regarding the following issues;

WE PROCESS YOUR PERSONAL DATA FOR THE PURPOSES AND LEGAL REASONS EXPLAINED BELOW:
In accordance with Articles 5 and 6 of the Law on the Protection of Personal Data,
in particular;
In order to process personal data belonging to the parties to a contract, provided that it is directly related to the establishment or execution of a contract;
In cases where data processing is mandatory for the establishment, exercise or protection of a right,
In cases where data processing is mandatory for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the relevant person,
In cases where public health is protected, preventive medicine, medical diagnosis, treatment and care services are carried out, and health services and financing are planned and managed.
YOUR PERSONAL DATA THAT MAY BE PROCESSED:
Your name, surname, TR identity number, passport number or temporary TR identity number if you are not a Turkish citizen, place and date of birth, marital status, gender information, and your submitted TR identity card or driver's license photocopy,
Your communication data such as your address, telephone number, e-mail address,
Your financial data such as your bank account number, IBAN number,
Your health and sexual life data obtained during the execution of your medical diagnosis, treatment and care services, such as your laboratory and imaging results, test results, examination data, prescription information that you provide for the purpose of tracking in your file,
The answers and comments you share for the purpose of evaluating our services,
Your closed circuit camera system image and audio recordings taken during your visit to our hospitals,
Voice conversation records kept if you contact our Call Center,
Your data regarding private health insurance for the purpose of financing and planning health services, Your Social Security Institution data,
Your vehicle license plate data if you use the parking lot and valet service,
Your navigation information obtained during the use of our website and mobile application, your IP address, browser information and medical documents, surveys, form information and location data that you have submitted with your own consent.
YOUR RIGHTS REGARDING YOUR PERSONAL DATA:

In accordance with Article 11 of the Law, you can apply to the Human Resources Department at Etimesgut or Pursaklar in writing with documents proving your identity or to the e-mail address kvkk@alifesaglikgrubu.com.tr to learn whether your personal data has been processed and if so, to request information about it, to learn which of your personal data has been processed, the purpose of processing and whether it has been used in accordance with its purpose, whether the information has been transferred to third parties domestically or abroad and, if so, to learn these third parties or categories of persons; You have the right to request that your personal data be corrected if it is incomplete or incorrect, updated if it changes, that your personal data be deleted or destroyed if the reasons requiring the processing of your personal data are eliminated, that the correction/deletion/destruction processes be notified to the third parties to whom your personal data has been transferred; to object to the emergence of a result against you by analyzing the processed personal data exclusively through automatic systems, and to request compensation for the damages in case you suffer damages due to the processing of personal data in violation of the legislation.
